Our Program
Champ Homes provides supportive, sober, transitional housing to individuals aged 18 and older on Cape Cod who are homeless or are at risk of homelessness. It is a community of people who benefit from the care, compassion, and the support of others. The organization serves individuals who are struggling with housing instability due to a variety of reasons including the current housing crisis.
Transitional Housing is an intermediate step between emergency shelters and permanent housing. It is more long-term (generally 2 years) and service-intensive than homeless shelters.
Participants work closely with a team of care coordinators to navigate continuing education, access healthcare, housing applications, obtain employment, and learn how to advocate for themselves.
Admission Criteria
To be accepted to our program, individuals must meet the following criteria and abide by our Participant Handbook.
Homeless
Currently living on the street, in a vehicle, in emergency shelter
Couch surfing, or hotel
Close to eviction
Prison
Medical/Mental Health History
Medical History; Diagnosis and medications
Mental Health History; Diagnosis and medications
Multiple physical and/or psychiatric concerns would need to be evaluated prior to admission
Sobriety
Minimum length of sobriety to be discussed at initial interview.
Criminal History
A CORI report is required at time of intake. If you have been charged/convicted of any of the following within the last 3 years you are ineligible for admission:
Assault & Battery charges
Domestic Violence charges (active no contact or stay away orders)
Level II & Level III sex offense charges
Trafficking charges
Financial
Ability to pay weekly Community Fee of $125.00
Sustainable income or ability to obtain sustainable income within 2 months from a secured funding source (family, agency, etc.)
If currently housed with another organization (that is not an emergency shelter) you are not considered currently homeless.
